What to bring
- Your member card — on your phone or printed, either works.
- Your member number, if you have it written down somewhere.
- A photo ID, so the front desk can confirm the card is yours.
The front desk confirms your membership with us before you are seen. You pay the member price for the service at the visit.
Are you a clinic, lab or pharmacy?
Providers apply to join the network and every application is reviewed by a person. There is no automatic approval.
